The first weeks after a hospital discharge are the most fragile stretch of a recovery. New medications, follow-up appointments, movement restrictions and reduced strength all arrive at once — usually on the same afternoon someone is dropped at home with a folder of instructions.

Post-hospital care puts someone in the house through that window. Our caregivers help with the practical work of recovering: getting around safely, keeping to the medication schedule, preparing meals, getting to follow-up appointments and keeping the household running.

We work alongside — never in place of — the home health nurses, therapists and physicians directing the medical side of recovery.
