Post-Hospital Care in Bolingbrook

Most readmissions happen in the first weeks home, and most of the causes are ordinary: a medication taken twice or not at all, a fall on an unsteady walk to the bathroom, an infection nobody spotted early, a follow-up appointment missed for want of a lift.
